1. Self attested copies of all documents.
2. Original form print out(2 Copies).
3. Passport size photograph(recent)(5 copies).
4. Photocopies of Aadhar Card, Domicile Certificates(2 copies each).
5. Photocopies of 10th Marksheet and Certificate(2 copies each).
6. Photocopies of 12th Marksheet and Certificate(2 copies each).
7. Photocopies of Graducation, Last examination Marksheet(2 copies each).
8. Photocopies of Higher Sports Achievemnt Certificate(2 copies each).

Note: Incomplete Application form and without above enclosures as mentioned shall be rejected.

After filling the online form, candidates have to take a print out of the form. Candidates should ensure that self-attested documents related to the course for which he/she is filling the form are sent by post (2 copies for all the documents).
